Does anyone know how many, and which episodes made use of Elfman's theme within the show itself, rather than just in the credits?

I think alot of the earlier episodes do. It's the full on 89 theme too!! :D

Elfman's theme is played in these episodes at these noted moments...

"On Leather Wings" - Once Batman reaches the chemical factory (or where-ever Man-Bat first struck) and fires his grappling gun and goes ziplining.

"Nothing to Fear" - When Batman gets jumped by The Scarecrow's goon in the bank vault and also later in the episode when Batman escapes from the fear-gassed party guests in pursuit of the The Scarecrow.

"The Last Laugh" - After Batman kills Captain Clown ("KILLS CAPTAIN CLOWN!") and chases The Joker down the conveyer belt (perhaps the best use of it).

I think there may be another instance or two... but I'm pretty sure these are it. I know them well because those moments caught my fancy as a kid. For whatever reason, the altered arrangement of Danny's theme in the opening and ending of the show didn't catch my notice as being his theme until years later. I thought it sounded similar but wasn't the same!  :o  ???
"There's just as much room for the television series and the comic books as there is for my movie. Why wouldn't there be?" - Tim Burton

There is an extremely subtle version in Feat of Clay part 1 during the scene where Batman chases and then interrogates Bell with the Batwing.
Why is there always someone who bring eggs and tomatoes to a speech?
