I always assumed the hook went through his jacket collar.

Paul is right. I watched it on WGN, at the sports bar of all places, and you can clearly see the hook coming out of his collar. By the way, the version WGN aired was the one shown on CBS with many edits, including: Nick's "no sh*t" line muted with Elfman's decending cue sounding clear as day, Jack's "Jesus!" line shortened to "Jeez!" and The Joker's "son of a bitch" line replaced with "son of a BATCH."
